DescriptionThe Ministry of Health funds 248 units of housing with supports for people who have been homeless and are living with a mental illness * Services offered: 
 
* Geared to income housing 
* Support, advocacy, care planning and support, life skills teaching 
* Community integration and social and recreational activities 
* Counselling and money management 
* Assistance with employment, education, volunteer opportunities 
* Several types of housing with varying levels of support
HoursAdministration Mon-Fri 8:30 am-4:30 pm * HOMES program staff on duty or on call Mon-Fri 24 hours

EligibilityAges: 18 year(s) and up 
 
For adult individuals 18yrs of age an up who are homeless/at risk of being homeless and who are living with mental health problems.
ApplicationInitial phone call to John Lee, Director, will screen for eligibility (re: homelessness/mental illness) * When units are available, person referred will be seen for Intake interview and then matched with appropriate housing.
