Description & Services
DescriptionAssertive Outreach Team 
A multi-disciplinary team engages with individuals living with mental illnesses, and who are living in shelters or are homeless. We help individuals connect to income sources, health care, housing and other support services. 
 
Case Management 
Public Health Nurses work with adults who are who are diagnosed as living with a serious mental illness to promote the achievement of optimal health and well-being and to increase independence and participation in community life. 
 
Referrals to either the case management or the street outreach services can be made by calling IntAc 905-528-0683.  
The Community Mental Health Promotion Program and Street Outreach service does not provide mental health crisis services - for crisis services contact COAST 905-972-8338 or the Police (911).  
For Community Treatment Order Referrals contact 905-388-2511 ext 3621. 
For Mental Health Court Support Service or Canadian Mental Health Association Supportive Housing and Community Support contact 905-521-0090.
