Description & Services
DescriptionThis multidisciplinary/multi-agency team provides case management and street outreach services * Case management services with a psychosocial rehabilitative/recovery focus are available to adults living with a diagnosed serious and persistent mental illness and in stable housing * Street Outreach services are available to adults who are experiencing homelessness.  
 
Services are offered to people where they live: in private homes, residential care facilities, hostels or on the street. The team works together with clients to support them in accessing social, health and housing services.  
 
Referrals to either the case management or the street outreach services can be made by calling IntAc 905-528-0683.  
The Community Mental Health Promotion Program and Street Outreach service does not provide mental health crisis services - for crisis services contact COAST 905-972-8338 or the Police (911).  
For Community Treatment Order Referrals contact 905-388-2511 ext 3621. 
For Mental Health Court Support Service or Canadian Mental Health Association Supportive Housing and Community Support contact 905-521-0090.
