DescriptionThe Schizophrenia & Community Integration Service (SCIS) is part of the Mental Health & Addiction Program at St. Joseph's Healthcare Hamilton. Our inpatient units and SCIS executive are located at the West 5th Campus; most outpatient services are located in the Hamilton and surrounding communities.  
 
We offer a comprehensive range of specialized mental health care to adults with schizophrenia and related psychotic disorders. Whether an individual is experiencing a psychotic episode for the first time, or is living with recurrent symptoms, they can expect support 
to manage symptoms and regain/improve their ability to acheive goals and live in the environment(s) of their choice. Multi-disciplinary teams work in partnership with clients, caregivers and other health care providers. Active consultation, assessment, treatment and rehabilitation are integral to the inpatient, transitional, outpatient and outreach components of our service. 
 
Inpatient Services 
There are three specialized schizophrenia inpatient units at the West 5th Campus which provide assessment, treatment and rehabilitation for people experiencing acute symptoms. We accept referrals through an Intake Coordinator from: (i) psychiatric services in the regions we serve (Halton, Brant, Haldimand-Norfolk, Niagara) (ii) the Psychiatric Emergency Service (St. Joseph's Healthcare Hamilton, Charlton Campus (iii) general (acute) psychiatric units.  
 
* Schizophrenia-C2 
This inpatient unit provides specialized assessment, treatment and rehabilitation for adults with acute schizophrenia/related psychotic disorders. 
 
* Schizophrenia-D2  
This inpatient unit provides specialized assesment, treatment and rehabilitation for adults with complex needs in several areas (psychiatric, medical, legal, psychosocial and behavioural), which may require more intensive supports, discharge planning and follow-up. 
 
* Schizophrenia-A2  
This inpatient unit provides specialized assessment, treatment and rehabilitation for adults with acute schizophrenia/related psychotic disorders. 
 
Outpatient Service 
The outpatient component offers specialized programs uniquely designed to explore a wide range of treatment options and alternative rehabilitation strategies for individuals needing expert treatment at any stage of illness, including transitioning from hospital to independent living in their preferred community environment.  
 
Our outpatient component includes: 
- Transitional Outpatient Program of the Schizophrenia Service (TOPSS):  
A transitional team provides support to vulnerable individuals moving from an inpatient to an outpatient/community setting  
- Community Schizophrenia Service (CSS):  
A community based specialized outpatient program that provides diagnostic clarification, consultation, assessment, shared care and individualized follow-up to individuals referred from the Schizophrenia & Community Integration Service's inpatient component and regional/community partners  
- Cleghorn Program:  
Provides rapid and specialized assessment, treatment and rehabilitation for people experiencing a first episode of psychosis, based on principles of recovery and partnerships with clients and their families. The Cleghorn Program is available to anyone living in the City of Hamilton aged 14 to 35 years old who may be experiencing their first episode of psychosis AND have never been treated for psychosis, or have been treated for six months or less. Extended consultation are also available for individuals living in our LHIN area. Family doctors, mental health professionals, teachers, school counsellors, social workers or family members are welcome to contact us about someone they are worried about. 
-Centralized Rehabilitation:  
Comprised of a number of programs geared to assist individuals with a psychotic disorder both in and out of hospital through gaining or regaining valued roles through evidence-based approaches in psychiatric rehabilitation. Programs include: Tobacco Addiction Recovery Program (TARP); Vocational Rehabilitation; Recreation Therapy; Individualized supportive health and wellness counselling; Inspiration Place (clubhouse); Rainbow's End Community Development Corporation 
-Assertive Community Treatment Teams (ACTT):  
Four ACT teams serve Hamilton, Haldimand-Norfolk and Brant County (including the Six Nations Reserve) by providing intensive community-based treatment, rehabilitation and supports (including peer support) to the most seriously mentally ill. ACT teams also support individuals by assisting with medication, housing, finances, employment and life skills. Treatment is considered assertive because staff work with individuals in their home, place of work and other chosen environments.  
-Homes for Special Care (HSC). This is part of a long standing province-wide residential care home program for adults with serious mental illness, which assists people to explore and fulfill life expectations beyond psychiatric stabilization and maintenance of physical health. There are 14 independently owned homes located in urban and rural areas across Hamilton, Haldimand and Brant, each regulated and licensed by the Ministry of Health and Long Term Care. Operations and staffing are monitored by interdisciplinary clinical staff from the local HSC Program Office. This program also oversees two Skills Centres.  
-Community Mental Health Service Coordination/Community Treatment Order (CTO) Coordination Program:  
Provides services for the Hamilton and LHIN 4 region by: developing CTO protocols; facilitating timely discharge and follow up for clients who meet the criteria for a community treatment order; ensuring legislative requirements are met; providing education to stakeholders; collecting and submitting data to Ministry of Health and Long Term Care. 
-Public Health Services -City of Hamilton:  
Public Health Nurse(s) provide outreach services to seriously mentally ill homeless in the Hamilton community.  
 
Education 
Education is provided to: 
* Clients, families and caregivers about the illness, treatment options, rehabilitation plans, community resources, follow-up, meaningful activity and employment. Specially trained staff offer a Family Education Program to families interested in learning about schizophrenia and tips for supporting the recovery of loved ones. 
* Staff by providing ongoing educational opportunities that reflect current thinking in schizophrenia treatment; for example education about schizophrenia, recovery, suicide prevention; cognitive behavioural strategies. 
* Student learners by offering quality learner placements for all disciplines that foster specialists. Staff contribute to student learning through supervising and tutoring, and a number hold academic appointments with McMaster University, Mohawk College and other post-secondary institutions. 
* Other health care providers at St. Joseph's and in the community. 
* Community-at-large
