DescriptionThe Seniors Home Support Program provides on-going support to seniors with low incomes so as to enable them to maintain their independence and remain in their homes.  
 
The program provides: 
* Housing assistance for seniors at risk of eviction, living in a shelter, living in unsafe housing 
* Foot care 
* Oral health care 
* In home visiting nursing and primary care 
 
Additional supports include group sessions on health prevention measures (for example falls prevention, healthy eating) and assistance linking with community services.
