DescriptionA collaborative program among partners which include St. Matthew's House, Hamilton Urban Core Community Health Centre and VON Hamilton * The program focuses on decreasing the number of older adults, including immigrants and refugees, who are at risk of homelessness or are homeless * The program improves access to and coordination of health services and supports including community outreach, in home nursing visits, oral health care and foot care services, consultations and education.
