DescriptionThe goal of the Building Health Care Capacity Initiative is to ensure that individuals with developmental disability, including those with mental health disorders or significant behaviour or emotional difficulties (dual diagnosis) have access to health care supports which meet their unique needs. The Health Care facilitator works with health care professionals, developmental service providers, family members and individuals to ensure access to health care supports. This includes assisting individuals find health care resources in their community.
