DescriptionPassport is a Ministry of Community and Social Services initiative. Through Passport, individuals with a developmental disability who have left school can receive funding for activities that encourage their personal development and help them achieve their potential. 
Passport is administered by Contact Hamilton. For information and an application package call: 905-570-8888 or visit their website. 
 
Catholic Family Services can assist individuals with the development of service plans, provide case management, act as a transfer payment agency and provide Human Resources information related to hiring, performance monitoring, payroll and other employer/employee relationship issues.

EligibilityPassport is intended for residents of Ontario that have a confirmed developmental disability and: 
 
* Have left school and would benefit from supports that will help them participate in recreational, vocational or educational opportunities in the community 
* Have been waiting for service and are in need of community participation supports 
* Are not receiving Ontario Disability Support Program (ODSP) employment supports 
* Participants in Passport can live independently or in supportive living. Priority will be given to applicants who are living at home with their families
ApplicationDevelopmental Services Ontatio / Hamilton Niagara Region is responsible for administering Passport for residents of Hamilton. Individuals and families interested in Passport can call Developmental Services Ontario / Hamilton Niagara Region at 905-297-5604 or 1-877-376-4674. DSO staff will assist with the application process.
