DescriptionPassport is an initiative of the Ministry of Community and Social Services for people with a confirmed developmental disability that have left school and want to find more ways to participate in their community. Through Passport, participants can receive funding for activities that encourage their personal development and help them achieve their potential.
HoursMon-Fri 10 am-3 pm, or based on individual needs

EligibilityPassport is intended for residents of Ontario that have a confirmed developmental disability and: 
 
* Have left school and would benefit from supports that will help them participate in recreational, vocational or educational opportunities in the community 
 
* Have been waiting for service and are in need of community participation supports 
 
* Are not receiving Ontario Disability Support Program (ODSP) employment supports.  
 
Participants in Passport can live independently or in supportive living. Priority will be given to applicants who are living at home with their families.
ApplicationDevelomental Services Ontario (DSO) is responsible for administering Passport for residents of Hamilton. Individuals and families that are interested in Passport can call DSO at 905-297-5604. DSO staff will send out an application form and related Passport information. A follow up appointment can be set up to address any questions relating to Passport and / or to explore other areas of need. Individuals and families can also choose to download the Passport application form and related information directly from http://www.contacthamilton.ca.
