| Description | Through individual and group education, St. Joseph's Adult Diabetes Program aims to assist people living with diabetes both to manage their condition and achieve a healthy lifestyle. A team of nurses, dietitians, social worker and diabetes specialists provide education, assessment, and follow-up to individuals living with diabetes The program consists of three phases: * Education - Focusing on self-management, clients are encouraged to attend various educational sessions. These sessions include: Living Well with Diabetes, Carbohydrate Counting, Eat Smart for your Heart, Label Reading, Eating Out, Craving Change, Take Charge and Insulin Management. Diabetes Classes are available to anyone in the community living with diabetes and a physician referral is not necessary. * Assessment - A nurse and dietitian assess and outline the individual health and social needs of the client. In collaboration with the client, mutual goals are set related to nutrition and weight management, blood sugar control, blood pressure targets, smoking cessation, eye and foot care. *Treatment and Support Pre Diabetes Program - This program is a series of four education classes. These classes provide information on how to lower your risk of developing Type 2 Diabetes and maintaining a healthy lifestyle. A family physician referral is necessary to attend these classes. | |||
