DescriptionThe Crisis Outreach and Support Team provides the following service: 
 
* Assists individuals who are in crisis and have a mental health concern and be supported in the environment of their choice 
* Supports caregivers including family members in managing acute mental health concerns and crisis through education and problem solving approaches 
* Links individuals during and following a crisis with the necessary community resources and support systems, including peer support, in order to prevent further crises 
* Offers these interventions and supports anywhere within the City of Hamilton 
* Acts as a resource for other communities within the Central-West Health District interested in developing similar programs 
* Mental health concerns include mood disorders, psychosis, addiction and mental health concerns, anxiety disorders, dual diagnosis, post-partum, personality disorders, and children's mental health concerns, etc.
HoursAdministration Mon-Fri 9 am-5 pm * Mobile Service 8 am-12 midnight * Crisis Line 7days/week, 24 hours

EligibilityAdults, Seniors, Youth, Children * COAST has a child/youth team as well as an adult team * program provides service to all age groups experiencing a mental health crisis
ApplicationAgencies can: 
 
a) Obtain crisis alert forms from the office and have them on hand to fax to the office any time 
b) Call the business line. Clients experiencing a mental health crisis can call the crisis line 24hrs/7days week or the police (911).
