| Description | The Geriatric Psychiatry Program at St. Joseph's West 5th Campus provides acute mental health services for older individuals with: * Behavioural/psychiatric mental health problems associated with cognitive impairment * Late onset psychiatric illness * Longstanding psychiatric disorders with age-complicated dysfunction * Medical and functional impairment with associated psychiatric and behavioural dysfunction * Alcohol and substance misuse * Families/caregivers who provide care Service components * Acute mental health inpatient care * Outreach services * Education of care providers * Ongoing program development * Evaluation and research of health services and policy development Outpatient Clinics * Brant Geriatric Outreach Program * Niagara Geriatric Outreach Program * Halton Geriatric Outreach Program | |||
