| Description | This support service provides a regularly planned "day out" for elderly and disabled adults in a structured, group setting. Programs are designed to meet a variety of needs and interests and assist individuals to achieve and maintain their maximum level of functioning, postpone or prevent institutionalization and to provide respite for caregivers. Services provided include a full course hot lunch, personal care assistance and medication reminders. Transportation may also be arranged and provided by the program. The variety of daily programs and activities are designed to promote an individual's cognition, orientation and physical functioning as they participate within a social network of friends, become part of a group, participating in meaningful, useful activities. | Eligibility | Adult Day Services are intended for the elderly who may be experiencing one or more of the following: -Frailty -Have a progressive cognitive disorder (such as Alzheimer`s Disease) -Have Dementia or an acquired brain injury Be socially isolated or need extra support, due to a medical conditions such as Stroke, Parkinson`s Disease, Multiple Sclerosis, or Diabetes. Clients must be assessed by the CCAC to determine an applicant's eligibility to attend. | |||
