| Description | The Healthy Babies, Healthy Children (HBHC) program is a home visiting program which supports pregnant women and families with children up to six years of age * The goal of the program is to ensure that all children have the healthiest start in life possible * The HBHC program: * Helps families find and access community services and supports * Helps families who may be living in difficult or stressful situations and ensure that they get the support and services that they need * Helps strengthen parent/child relationships and build positive parenting skills through education, support and linking with community resources * Supports the healthy growth and development of children from birth to age six Services available through the Healthy Babies, Healthy Children Program are: * A postpartum telephone call and home visit by a Public Health Nurse * Referral to a Public Health Nurse who can provide support and health teaching home visits during pregnancy and from birth to six years of age * Access to health information through the Health Connections Phone Line * Assistance in connecting with community resources and services both immediately and long term * Referral to a Family Home Visitor who can work with parents and their children to promote growth and development and healthy parent/child relationships * Referral to prenatal programs and nutrition support during pregnancy Families may self refer or be referred by a service provider with their consent. Services are available in many different languages. Healthy Babies, Healthy Children is an initiative of the Ministry of Children and Youth Services |
