Description	Care is provided in the home for the adult with a long-term disability while the family caregiver takes a break * A trained worker provides companionship and supervision, help to do enjoyable activities in the home, and assistance with personal care excluding bathing * Priority is given to adults with progressive memory loss such as Alzheimer Disease
Hours	Mon-Fri 8:30 am-4:30 pm

Eligibility	Adults; Seniors     Family caregivers who are caring for adult family members with dementia or with other long-term care conditions at home

Fees	Service - $5.00 per hour charge
