DescriptionThis is a day program offered within the Church of the Nativity (formerly St. Mary's Anglican Church) which assists seniors and adults with diminished abilities, including dementia, to maintain their independence in the community * The SAM program offers a one to five day regimen of activities for registered clients * The main focus is on activation, wellness, and integration * Major services include: 
 
* A variety of activities which foster independence  
* Supervision and respite care as needed  
* Support for both clients and caregiver  
* Nutritious meals and refreshments 
* Supported Conversation Group for survivors of stroke with aphasia  
 
SAM staff will initiate appropriate transportation arrangements as needed
